CHARLOTTE SHOOTOUT INDIVIDUAL PLAYER REGISTRATION & WAIVER
On or before, Friday, January 14, every player who participates in the Polar Bear High School Championships is required to complete the online registration process prior to taking the field on Saturday. If you do not register, you can not play.
The online waiver provides medical info to our trainers and serves as a liability release.
There is NO cost to complete this form.
