Online Team Check-In

We are pleased to introduce our Online Check-In capability. Using this feature will save you tons of time! All Tournament Teams are required to use the Online Check-in capability - the cost is $20 per team for the 2018 United Cup.

Online Check-in is now open and will close on April 10th at the End of the Day!

Please follow the instructions below for Online Check-In:

Login to your account - Send an email to Tournaments@CharlotteUnited.com if you have a problem logging in.
Click the EZ Check-In link to the right of the event
Pay the $20 Online Check-In fee (Discover, MasterCard & Visa are accepted)
On the Receipt Page, click the "Return to Account Management" button
Click the EZ Check-In link again
Load documents from your computer or mobile device – be sure to load all three documents (All documents must be a JPG, GIF, or PDF File)
Once all documents are loaded, click the Submit for Approval button
We will respond with your approved documents via email within 1 Business Day

Required Documentation

Documentation Required for Check-In: Below are documents required to be submitted for Online Team Check-in!

Roster:
- US Club: US Club Roster (Original + 4 Copies)
- Non-US Club: Tournament Team Roster Form (Original + 4 Copies) - This is an Excel file! (No USYS Roster - NCYSA, SCYSA, Etc.)
Player Liability Release Form – Parent/Guardian Signature for Each Player!
Age/Medical Waiver Certification Form - Rostered team official must fill out and sign.
NOTE: All Teams MUST have available on the sidelines during ALL of their games US Soccer Club Medical Release Form and proof of age/US Club Cards for any and all players. Proof of age comprises an official id (e.g. birth certificate, passport, drivers license, US Club player cards) or a NCYSA Team Information form (NCYSA Player Cards are not a valid form of proof of age).

Tournament Insurance: Charlotte United FC has secured supplementary insurance for tournament attendees. Your team does NOT need to apply for team insurance through US Club Soccer.
