NCYSA Approval Form: The NCYSA will provide your team, upon request, with a Team Player Information document. The request must be submitted by a team rostered official by following the instructions below:

• Go to NCYSA website www.ncsoccer.org
• Click on Tournaments
• Click on Perm to Travel (Permission to Travel)
• Click on Online Notification to Travel (near the bottom of the page)
  
• Proceed to ‘Team Information Form’ (near the bottom of the page)
• Complete requested information
• Click Submit Team Information (there is no fee for the request)
• In the drop down, choose "Request to attend a Non-Sanctioned Event"
• Click Fill out an e-Travel Form (near the bottom of the page)
• Enter tournament information.
• Click Submit e-Travel Form Information
• You will receive an email with the information required to print the "Team Information Form"

Tournament Insurance: North Meck SC has secured supplementary insurance for tournament attendees. Your team does NOT need to apply for team insurance through US Club Soccer.
